ELC Entrance Requirements
The following items must be submitted to the ELC office before a child can begin in our program:
- Student registration form
- Pre-Admission Form (DHR-CDC-739)
- Parent/Center Agreements
- Current immunization record
- $100 registration Fee (renewable each year)
- Tuition
- Lunch form
- Photography permission form
- Evacuation Plan
